1. ADR — परिभाषा और महत्व
Adverse Drug Reaction (ADR) वह हानिकारक या अनपेक्षित प्रतिक्रिया है जो किसी दवा के सामान्य खुराक पर होती है। यह रोगी की स्वास्थ्य स्थिति और उपचार की सफलता पर प्रभाव डाल सकती है। WHO Pharmacovigilance के अनुसार, ADRs का समय पर पहचान और प्रबंधन महत्वपूर्ण है।
2. ADR के प्रकार
- Type A: Dose-related; predictable (उदा. Warfarin से रक्तस्राव)
- Type B: Unpredictable, allergic/idiosyncratic (उदा. Penicillin से एरिथेमा)
- Type C: Chronic use से होने वाली प्रतिक्रियाएँ (उदा. Corticosteroids से osteoporosis)
- Type D: Delayed reactions (उदा. Chemotherapy के बाद secondary malignancies)
- Type E: End-of-use withdrawal reactions (उदा. Benzodiazepine withdrawal)
- Type F: Failure of drug effect (उदा. Antihypertensives पर BP uncontrolled)
3. लक्षण और वर्गीकरण
- त्वचा: Rash, hives, erythema, exfoliation (Dermatology page)
- श्वसन: Wheezing, dyspnea, throat tightness
- जठरांत्र: Vomiting, diarrhea, abdominal pain
- स्नायविक: Headache, dizziness, seizures
- हृदय: Arrhythmia, hypotension, hypertension
4. निदान और परीक्षण
- Naranjo Scale for causality assessment
- WHO-UMC Causality Assessment
- Lab investigations: CBC, LFT, KFT, Electrolytes, ECG
- Drug levels for narrow therapeutic index drugs
- Skin biopsy for severe dermatologic ADRs (SJS/TEN)
5. उपचार और दवाएँ
5.1 Urticaria / Rash
- Cetirizine 5–10 mg PO once daily
- Loratadine 10 mg PO once daily
- Severe itching: Diphenhydramine 25–50 mg
5.2 Angioedema
- Stop suspected drug immediately
- Antihistamines ± corticosteroids
- Airway compromise: IM Epinephrine 0.3–0.5 mg (adult)
5.3 Anaphylaxis
IM Epinephrine 0.3–0.5 mg (adult), Oxygen therapy, IV fluids, Antihistamines, Corticosteroids, close monitoring.
5.4 GI Reactions
- Anti-emetics: Ondansetron 4–8 mg IV/PO
- Fluid replacement: Oral/IV
- Proton pump inhibitors: Omeprazole 20–40 mg
5.5 Hepato-renal ADRs
- LFT/KFT monitoring
- Stop hepatotoxic/nephrotoxic drugs
- Supportive care: hydration, electrolyte management
- Referral to specialist if severe
5.6 CNS ADRs
- Seizures: Lorazepam 4 mg IV
- Dizziness/Vertigo: Hydration + symptomatic care
- Monitor for neurotoxicity with chemotherapeutics
6. खाद्य और जीवनशैली परहेज
- Grapefruit से बचें (CYP3A4 mediated interactions)
- Alcohol से बचें
- High-fat meals से कुछ drugs का absorption प्रभावित हो सकता है
- Hydration और balanced diet बनाए रखें
7. आपातकालीन प्रबंधन
- Airway, Breathing, Circulation stabilization
- IM Epinephrine for anaphylaxis
- Hospital admission for SJS/TEN, angioedema, hypotensive shock
- Supportive care: fluids, oxygen, ICU monitoring
8. रोकथाम और रिपोर्टिंग
- Maintain ADR card for patient
- Medication reconciliation at every visit
- Educate patients on early signs of ADR
- Report to National ADR Portal India
