परिभाषा / Definition: Melena (मेलिना) वह स्थिति है जिसमें मल का रंग काला, टेरी और चिपचिपा होता है, कारण होता है पचा हुआ खून (digested blood) मल में आना। यह आमतौर पर Upper GI bleeding का संकेत है।

1. परिचय (Introduction)
Melena (मेलिना) एक clinical sign है, disease नहीं। यह दर्शाता है कि आपके पाचन तंत्र (Gastrointestinal tract) के upper हिस्से में कहीं bleeding हो रही है।
MBBS और MD स्तर पर इसका differential diagnosis करना ज़रूरी है क्योंकि इसका कारण साधारण gastritis से लेकर life-threatening variceal bleed तक हो सकता है।
2. पैथोफिजियोलॉजी (Pathophysiology)
जब blood upper GI tract में आता है, gastric acid (HCl) और enzymes उसे digest करते हैं। Hemoglobin टूटकर hematin में बदलता है, जो काला रंग देता है। Transit time slow होना चाहिए ताकि यह change हो सके।
- Melena दिखने के लिए कम से कम 50–100 ml blood loss होना ज़रूरी है।
- Fast transit time → fresh blood (hematochezia)।
- Slow transit time → black tarry stool (melena)।

4. लक्षण (Symptoms)
- काला, टेरी, चिपचिपा मल
- तेज़ दुर्गंध
- थकान, कमजोरी
- चक्कर, बेहोशी
- Vomiting with blood (hematemesis) – अगर upper GI source है
5. जटिलताएँ (Complications)
- Severe anemia
- Hypovolemic shock
- Multi-organ failure
- Death (अगर इलाज न मिले)
